The University of Maryland Medical Center (UMMC) is a world-renowned tertiary and quaternary Advanced Heart Failure Center.
The Heart Failure Clinic in the Coordinated Care Center at UMMC is a transition of care outpatient service that cares for the heart failure population. Emphasis is placed on safe transitions of care post-discharge from hospital admission for heart failure and the titration of guideline-directed medical therapy for those with heart failure. We seek an Advanced Practice Provider who will enjoy caring for this fragile population. Ideally, the candidate will have one year of cardiology experience, including an understanding of EKG interpretation and hemodynamic monitoring.
Job Specifics include the following:
• The population includes patients who have heart failure with a variety of etiologies.
• The ability to work with a multidisciplinary team to address the social determinants of health that impact one’s ability to engage in medical care.
• Some familiarity with how patients need to manage inotropes on an outpatient basis.
• The clinical evaluation of patients includes an assessment of their volume status and perfusion.
• Outpatient monitoring of implantable pulmonary artery sensors.
• Patient and family education.
• Communication with providers, including attending pharmacists, physicians, residents, fellows, and other APPs.
• The Nurse Practitioner (NP) or Physician Assistant (PA) is responsible for diagnosing and treating patients in collaboration with physician colleagues.
• The Nurse Practitioner (NP) or Physician Assistant (PA) provides continuity of patient care by facilitating and coordinating communication between the health care team, the patient, and the patient’s family.
• The Nurse Practitioner (NP) or Physician Assistant (PA) serves as a link, integrating relevant research and best practices.
• The hours will be from 8 a.m. to 6 p.m. two days per week. The clinic does not need to be covered on holidays or weekends.
• Will include one week of on-call time for after-hour coverage of patients.
A Master of Science degree, with completion of an Adult or Family Nurse Practitioner Program or Physician Assistant’s Program. Preferably, the candidate will have one year of experience in an ICU. Cardiology experience is a plus.
