In total, our 141-bed facility welcomes over 3,000 patients every year. As the state’s largest provider of inpatient rehabilitation services, we offer some of the most advanced therapies for stroke, spinal cord injury, traumatic brain injury, orthopedic and sports injury, among many others.
General Summary
Under general supervision, provides consultation, education, public relations and patient assessment services in order to obtain admissions into post acute programs.
Principal Responsibilities and Tasks
- The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.
- Performs on-site detailed functional assessments on patients referred for post-acute services. Determines potential and appropriateness of post-acute services.
- Serves as a patient advocate to recommend appropriate level of care or alternate resources available to meet the patient’s assessed needs.
- Provides pertinent information and feedback to post-acute care staff regarding clinical and administrative issues.
- Works with patients and families to provide information and facilitate a smooth transition to post-acute services.
- Provides detailed assessments and treatment plan goals to facilitate reimbursement authorization from managed care contracts.
- Collaborates with post-acute sites and referral coordinators to insure smooth flow of patients from acute care and community settings to post-acute facilities.
- Collaborates with external case managers, medical staff, nursing, and social work to ensure continuity of patient care between community hospitals and post-acute care service sites.
- Provides educational services to the UMMS community and to third party payors regarding post-acute care service lines.
- Participates in public relations and marketing projects/programs for post acute services and promotes post-acute care services referral network in community hospitals and other healthcare organizations.
- Provides coverage for additional sites in post-acute services, as required, with cross trained capabilities in all areas.
- Educates healthcare staff in community hospitals on the comprehensive rehabilitation and long-term services provided by University of Maryland Medical System.
Education and Experience
- Graduation from an accredited school of Nursing is required. Bachelor’s Degree in Nursing is preferred.
- Current licensure as a registered nurse by the Maryland State Board of Examiners Nurses is required. Membership in the Association of Rehabilitation Nurses (ARN) and other professional organizations is preferred.
- Current valid Maryland Drivers License is required.
- Three years’ experience as a RN in a rehabilitation or long-term care setting is required. Experience as a Case Manager or Nurse Liaison is preferred.
Knowledge, Skills and Abilities
- Highly effective interpersonal and communication skills are required to interact effectively with patients, families, and other staff within the Hospital and at hospitals throughout the community.
- Must possess the ability to work collaboratively with other disciplines.
- Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient’s requirements relative to his or her population-specific and age specific needs.
Patient Safety
Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.
- Takes action to correct observed risks to patient safety.
- Reports adverse events and near misses to appropriate management authority.
- Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.
