What You Will Do:

University of Maryland Medical Center is currently seeking Community Health Workers to join our NEW Mobile Integrated Health Program. Don't miss out on this wonderful opportunity to be a part of a new team here at UMMC. 

This is a full time position - M-F day shift but does require rotating weekends once the program kicks off

The CHW - Join a dynamic partnership between the University of Maryland Medical Center and Baltimore City Fire Department with the Mobile Integrated Health (MIH) Community Paramedicine Program. Mobile Integrated Health bridges the gap between inpatient medical care and outpatient community based care. 



Principal Responsibilities and Tasks

The Community Health Worker (CHW) will be responsible for:

  • Educating patients and enrolling in the MIH Program
  • Schedule appointments, provide follow-up phone calls, coordinate care, and link patients to community resources
  • Provide outreach to patients and support the field-based in-home medical teams, as well as other duties

What You Need to Be Successful:

  1. High School Diploma or equivalent (GED) is required.
  2. Working experience providing outreach services.  Experience should include writing reports and maintaining records.
  3. Previous experience in care coordination or community health worker background. Understanding of medical terminology, a plus. 

Knowledge, Skills and Abilities

  1. Effective oral and written communications skills.
  2.  Ability to establish and maintain effective working relationships.
  3. Ability to handle sensitive and confidential matters with discretion and tact.
  4. May be required to use personal automobile while conducting official business.  Must possess a valid Maryland Non-Commercial Class C or Commercial Class B Driver’s license (CDL) if required to drive.
  5. Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.
