UM St Joseph Medical Group is currently seeking aProgram Consultant, Population Health(Ambulatory) to join our team located in Towson, MD. Ideal candidates will have experience working with physicians offices (Ambulatory). The Population Health Program Consultant is responsible for:
Managing all aspects U of MD St. Joseph Medical Group (UMSJMG) performance in population health programs for outpatient practices.
Tracking and reporting performance, educating providers and staff, and acting as the subject matter expert on all population health programs impacting UMSJMG operations.
Quality Performance Reporting & Improvement
Obtain and analyze timely metrics on performance in various programs such as MIPS, ACO, and other programs in order to identify potential risks for achieving quality and cost targets.
Develop and manage monthly dashboard for physician performance in various population health programs.
Consult with senior leadership and other administrative stakeholders regarding current state performance an identifying and managing opportunities for improvement.
Educate providers on program regulations and objectives through face-to-face interactions and development of training materials.
Determine best practices for incorporating population health metrics into workflows and share the information across UMSJMG.
Attend meetings, conferences, and other continuing education events to maintain knowledge of changes to existing programs and develop understanding of new programs.
Participate in UMSJMC/UMSJMG UMMS system-wide meetings and represent UMSJMG on teams tasked with developing UMMS population health programs.
Work with providers, practice staff, and Informatics team to continuously optimize workflows incorporating requirements of population health programs in which UMSJMG participates.
Review and discuss existing workflows with physicians in order to achieve clinical quality metrics.
Identify technical and operational obstacles to meeting UMSJMG objectives and work with key stakeholders to facilitate optimum office and EMR workflows to achieve performance targets and enhance efficiency.
Subject matter expert for systems used to report quality program metrics for UMJSMG; including Accountable Decision Support Platform (ADSP), ACO data, data from CMS sites, CRISP, etc.
Generate monthly dashboards and reports out of these systems and provide ad hoc reporting as requested by administration.
Assist in the resolution of system issues impacting quality reporting.
Ambulatory Population Health Program Leadership
Primary point of contact with representatives for population health programs (PCMH, QCN, CMS, etc.).
Interact with local/regional payor representatives for population health programs to discuss UMSJMG goals and performance.
Participate in monthly and quarterly meetings to review data and present feedback from providers. Discuss operational issues related to performance.
What You Need to Be Successful:
4 year / Bachelor's Degree is required.
Combination of relevant education and experience may be considered in lieu of degree.
Experience and Skills
2 - 4 years project management experience, to include leading strategic initiatives, data analytics and consultation is required.
5 years experience with large physician groups analyzing workflows and quality metrics is required.
2 or more years of experience with population health programs such as ACOs and PQRS is required.
