What You Will Do:

UM Baltimore Washington Medical Center is currently seeking a Transitional Nurse Navigator for our Case Management team, located in Glen Burnie, MD.

General Summary

The Transitional Nurse Navigator (TNN)  is responsible for seeking and implementing interventions, methodologies, and best practices for the high risk population. Using an evidence-based tool, the TNN:

  • Identifies and assesses the patient for priority needs, key areas for Needed support and services that contribute to readmissions
  • Develops care plans and when appropriate refers for services to assist in navigating barriers to care
  • Effectively collaborates and communicates with internal and external customers to maximize the patient’s potential for a healthy lifestyle evidenced in reduced Emergency Department visits and readmissions to the hospital
  • Effectively uses data responsibly in this role
  • This position is goal-oriented with a high level of accountability correlated to the reduction of readmissions, continuum of care, quality of care, and patient satisfaction.

What You Need to Be Successful:

  • Bachelor’s degree in nursing or related field required, Master’s degree preferred. 
  • Current Maryland State Licensure as a Registered Nurse.
  • Three (3) years of experience in discharge planning, case management or disease management required. 
  • Strong skills in patient education, coordination of discharge planning, and post discharge follow-up.
  • Proficiency in use and interpretation of word, excel and other data collection programs
  • Must be proficient in effective verbal and written communication; have strong organizational and problem-solving skills, the ability to perform multiple tasks with interruptions, and to manage time effectively in order to achieve simultaneous goals.
  • Must demonstrate a commitment to goals, teamwork, and standards of excellence.
