The Sr. Nurse Navigator provides and coordinates care to patients within the Baltimore Washington Medical Center on or consulted for the Palliative Care Service.
The Sr. Nurse Navigator serves as a fully effective and participative member of the Interdisciplinary patient care team.
The Sr. Nurse Navigator demonstrates and models the full range of nursing practices in both care provided and in participation in development of strategies to improve patient outcomes, define patient goals, satisfaction with service and utilization.
The Sr. Nurse Navigator has accountability to function as a team member to accomplish patient outcomes identified in the plan of care.
Patients served include pediatric through adult-geriatric.
-Graduation from an accredited nursing education program approved by the Board of Nursing or found by the Board to be substantially equivalent to the programs in Maryland.
-Currently licensed as an RN in the state of Maryland. BSN required.
-Five (5) years direct patient care experience as an RN required. Successful completion of a refresher course within the past six (6) months if nursing practice has not occurred within the past five (5) years.
-Two (2) or more years in Oncology, Hospice or Palliative Care required.
-Certified Hospice and Palliative Care Nursing (CHPN) is required within 2 years of hire and maintained throughout employment.
-Previous experience using a business office suite, such as Microsoft Office, is required.
-Current BLS certification or obtained prior to providing direct patient care is required.
