The Homecare Coordinator manages the care and service of patient populations being discharged to a homecare setting. He/ she works collaboratively with physicians and other members of the health care team to achieve the highest quality clinical outcomes with the most cost effective use of available resources. The Homecare Coordinator assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors and measures the effectiveness of interventions to meet patients’ treatment and transitional needs.

Home Health or home infusion (or one year of case management or discharge planning) experience (Preferred)
EDUCATION:
4 year/ Bachelor's degree in Nursing or Social Work (Required)
Master's Degree in Nursing or Social Work (Preferred)
CERTIFICATION/ LICENSURE/ REGISTRATION:
Active RN or Social Work license (Required)
