What You Will Do:

The Licensed Social Worker will provide psychosocial assessments for patients and families related to onolcology, hospitalization, and coordination of discharge planning with the interdisciplinary team, patients and families and the community.

Compliance:

PATIENT CARE:
Screen, identify and provide psycho-social assessment to patients and family members. Formulate, develop and implement a comprehensive discharge plan. Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation. Coordinate agency involvement in the patient's care both in the inpatient and outpatient settings.  Demonstrate an understanding and knowledge of counseling interventions.

RESOURCE UTILIZATION:
Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., adult and child abuse, guardianship. Maintain working knowledge of and liaison with community agencies and resources.

Coordinates discharge planning services, as well as addressing any barriers that may impact or delay the patients’ discharge plan. Provide consultative services to health care team members within the scope of care definitions as needed. Provide supervision to social work staff seeking advanced licensure or guidance in case management and discharge planning.



What You Need to Be Successful:

Bachelor's Degree: Social Work (Required)

Master's Degree (Preferred)

Certification / Licensure / Registration

*Licensed Bachelor Social Worker (Required)

Licensed Clinical/Licensed Graduate Social Worker (Preferred)

1-4 years hospital Social Work experience (Preferred)
