What You Will Do:

General Summary

Under direct supervision, performs variety of tasks involved in ensuring accurate and timely release of medical information to requestors. May be assigned to a combination of specialized functions.

Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

  • Receives incoming requests including opening mail, assist with walk-ins and telephone inquiries and retrieves facsimile inquiries. Date stamps all requests and highlights pertinent data to facilitate process of release of medical information in the ROI area.
  • Validates requests, authorizations and subpoenas for release of medical information according to established procedures.
  • Completes release of information requests including retrieving patient’s medical chart and returns chart, copies medical record accurately and correctly, and according to requests, established procedures, established standards of quality and productivity, and mails copies of same.
  • Invoices and transmits on a daily basis according to established procedures. Processes fee approvals according to established procedures.
  • Reports daily activity by data entering incoming requests into computerized tracking system including date received, date processed, and actions taken, and transmits log.
  • Performs quality checks on all work to assure accuracy of the release, confidentiality, and proper invoicing.
  • Provides excellent customer service including being attentive, eager to please, and respectful; ensures understanding of customer request and follow-through as promised.
  • Demonstrates excellence and accuracy in completing assigned responsibilities within deadlines.
  • Attends and participates in educational sessions and staff meetings as assigned.
  • Maintains confidentiality, security and standards of ethics with medical record information including during transport, storage and disposal.
  • Performs duties in accordance with established departmental production standards.
  • Performs related duties as required.

What You Need to Be Successful:


  • Completions of a high school level of course work with attainment of a high school diploma or a State High School Equivalency Certificate (GED) required.
  • Minimum of one (1) year medical records clerical experience performing CRT data entry and filing or related health care functions required.
  • Demonstrated knowledge and ability in the use of applicable personal computer systems and related software. Ability to learn specific software applications such as Pathway Image Manger, STAR and CharFlo required.
  • Demonstrated understanding of applicable medical terminology preferred.  Ability to learn medical terminology required.
  • Ability to operate routine office equipment such as photocopy and fax machines, PC’s and peripherals.
  • Effective oral communication skills are necessary to interact and work with various levels of hospital staff in a courteous and effective manner.

Working Conditions

  • Weekend, shift work, holiday, on-call and overtime may be required.
  • Work requires manual dexterity for sorting and assembling medical records and reports; computer
  • terminal work requires visual acuity and mental concentration.
  • Requires prolonged periods of sitting with a light amount of physical work, frequent lifting of maximum of 20 pounds, carrying and filing of medical records/documents weighing up to 10 pounds.
  • Work is performed in a normal office environment.
