What You Will Do:

General Summary

Under general direction, responsible for the overall coordination, provision, and continuity of services to individuals with a serious and persistent mental illness to enhance self-management skills and quality of life.  Within a strengths-based recovery model, provides an array of support services to assist individuals in achieving personal recovery goals and preferred lifestyles.   Assesses, plans, develops, implements and evaluates psychosocial interventions and supports.  Provides a wide range of services that include onsite and off site services assessing and coordinating internal and external services.

  Principal Responsibilities and Tasks

  1. Performs individual formal assessments that include risk/danger to self or others, symptoms and behavior, drug and alcohol abuse, natural supports, and community, social, and daily living skills to satisfy Medical Assistance, COMAR, and HCU program  admission requirements.
  2. Communicates to individuals significant to rehabilitation process. Recognizes the importance of families and the needs of families and attempts to engage them in this process as appropriate with the permission of the individual served. 
  3. Uses motivational interviewing and counseling skills, assists consumers to develop individual mental health recovery based overall rehabilitation goals that define the purpose and focus of program services and participation.
  4. Develops Individual Rehabilitation Plans (IRPs) every six months that include consumer focused, consumer determined SMART goals (six months), short term objectives, and on-site and off-site staff interventions and services.
  5. Identifies social, environmental and health care needs and helps members develop daily living skills.  Directly provides interventions such as skills teaching, counseling, roles-playing, problem-solving, and other forms of assistance in helping members overcome barriers to goal achievement.  Independently develops individualized natural supports and resources.
  6. Ensures that all documentation and clinical charts are continually up-to-date and meets organizational standards. Obtains documentation from other service providers as needed.
  7. Responsible for meeting billing/service goals, ensures accurate billing documentation, follows policies and procedures, and submits on schedule.
  8. Assists other team  staff  in the coordination of care and delivery of services. Works cooperatively and collaboratively  with the intake coordinator, benefits coordinator, residential rehabilitation staff, home health team and other division program staff in meeting the needs and goals of individual members.
  9. Attends all required and relevant training to remain current in best practices.
  10. Develops, implements, and evaluates class curriculum.  Documents member goals and objectives for each class curriculum.  Adheres to set schedule and curriculum of classes to ensure efficient and effective service provision.
  11. Provides crisis intervention and oversees the resolution of crisis for each individual served by the team.  Assists members to recognize antecedents to crisis and to develop coping strategies.
  12. Addresses positive and negative group and individual participation in classes in a manner that promotes dignity, comfort, learning, respect and is therapeutic.  Plans and conducts social activities around social goals and preferences of members.
  13. Maintains close communication around inpatient hospital stays and subsequent discharge planning.
  14. Works in coordination with the Home Health team to assist with coordination of care activities including scheduling of somatic appointments, reminders and when necessary, provides assistance in attending critical appointments or procedures
  15. Interacts with all individuals in a respectful, dignified, therapeutic, adult, engaging, and culturally sensitive manner using identified counseling and communication skills.

 



What You Need to Be Successful:


Education and Experience

  1. Bachelor’s degree in Human Services related field or equivalent combinations of education and experience from which comparable knowledge and abilities have been acquired.
  2. Four years experience performing mental health services, or equivalent, is required.
  3. Current, valid, MD state driver’s license is required.  No more than two points on the driving record per year permitted.
  4. Experience in curriculum development preferred.
