UM SJMC is seeking a Care Manager - Social Worker (Maryland Licensure; MSW) to provide psychosocial assessments for patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
This is a Weekend Day Shift: 8am - 4:30pm
Job Responsibilities and Accountabilities Patient Care: • Screen, identify, diagnose using DSM nomenclature, treat and manage mental health problems in patients and family members. • Formulate, develop, and implement a comprehensive psychosocial treatment plan. • Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation. • Coordinate agency involvement in the patients care in the inpatient and outpatient settings. • Demonstrates understanding and knowledge of counseling interventions through active participation in supervision. Resource Utilization: • Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., child abuse. • Maintain working knowledge of and liaison with community agencies and resources. • Maintain registry and tracking of patients referred to consultant treatment teams. • Coordinates discharge planning services, as well as addressing any barriers that may impact or delay the patients’ discharge plan. Provide consultative services to health care team members within scope of care definitions as needed.
